It might be worth checking to see that your valves are opening fully on the #4 cylinder. Since you swapped out heads, I'd assume you checked the valves for correct clearance on all cylinders but if #4 is has clearance that is excessive you could still see good compression and leakdown test results. Since the valves aren't opening enough (or at all) air/fuel mix won't get into the cylinder or exhaust can't get out.

You might also check that you are actually getting a spark at the #4 plug. Although you checked the cap and plug itself, the wire might be broken or grounding out.
