Shaft nut keeps stripping

So I bought a 2007 CSX 220 and it was dripping tranny fluid from the outbound shaft, I fixed that and about 30 hrs later the shaft retaining nut stripped (pic with the ruler). I replaced with all factory parts and tighten it as much as I could with the space and used lock tight. Today it spripped again. (Other pics). Leaving me to get towed in again.

My question is why is this happening. Is it just not tight enough or could a misalignment cause this (I donít feel or see one) Would a lock washer underneath the nut help?

Please help. Iím going to get another nut. Clean the shaft threads and try it again but want to know why this is happening.

I would suspect you have an alignment problem. Have you checked the engine alignment?

Suspect your shaft is done.

The alignment was off. I was able to adjust one side of the engine and got it very close but not perfect. The other side is rusted pretty bad but I’ve got some penetrating oil working on it.

Shaft is still strait as a arrow and luckily the threads are still good. Put the coupler on with a new nut just need to get that one side of the engine to drop and I should be back in business.

Thanks.
